I have two quick questions for you.

1. Is that a logical partition that I see XP and Vista installed on?

2. Is your XP entry showing in the boot display menu?

You will need the three files from XP copied to the root of drive C:
ntldr
NTDETECT
boot.ini

As per what jbullard is asking, is your drive D a logical or active primary partition?
